Adult Autism Waiver

RESIDENTIAL HABILITATION

Embolden Community Allies is committed to assisting adults with Autism in acquiring, retaining or improving communication, socialization, nutritional, self-care, and safety skills that are necessary to reside in the community. Our team is committed to providing residential habilitation services in fully integrated community locations in order provide our residents with opportunities for community participation, enhance adaptive skills in order to build their confidence and independence, and enable residents to exercise their of civic responsibilities.

 

 

Residential habilitation support services are designed to address the resident’s needs and goals (as documented in his/her ISP). Direct support staff shall be instrumental in:

 

  • Assisting in carrying out activities of daily living such as personal grooming and hygiene, dressing, making meals and maintaining a clean environment.
  • Assisting residents in developing and maintaining positive relationships with residents living in the same home and sharing responsibilities for shared routines such as: preparing meals, eating together, light cleaning, planning and scheduling shared recreational activities and other typical household routines.
  • Assist residents in developing practices that promote good health and wellness such as: nutritious meal planning, regular exercise, carrying out prescribed therapies and exercises, awareness and avoidance of risk including environmental risks, and responding to emergencies in the home and community such as fire or injury.
  • Assisting residents in scheduling and attending medical appointments, filling prescriptions and self-administration of medications, and keeping health logs and records.
  •  Assist residents in enhancing decision making and time management skills, including identifying options/choices and evaluating options/choices against personal preferences and desired goals.
  • Assisting residents in communicating with other service providers, caregivers, family members, friends and others face-to-face and through the use of the telephone, correspondence, the internet, and social media, consistent with the resident’s preferences or primary language/communication mode.
  • Transporting residents or assisting residents with using a range of transportation options including buses, trains, cab services, and joining car pools, etc.
  •  Assisting residents in developing and maintaining relationships with members of the broader community.
  •  Assisting residents in their civic responsibilities such as voting and serving on juries; attending public community meetings; to participate in community projects and events with volunteer associations and groups; to serve on public and private boards, advisory groups, and commissions, as well as develop confidence and skills to enhance his or her contributions to the community.
  • Encouraging personal interests such as hobbies, appreciation of music, and other experiences the resident enjoys or may wish to explore.
  • Participating in preferred activities of community life such as shopping or going to restaurants, museums, movies, concerts and faith-based services.

 

RESPITE (IN-HOME & OUT-OF-HOME):

Respite services provides planned or emergency short-term relief to waiver participants when the parent or legal guardian is temporarily unavailable to provide supports due to unforeseen circumstances. Services may be provided in the participant’s home or in one of our company residences. Embolden Community Allies is committed to ensuring the participant’s participation in community activities in order to facilitate social interaction.

Respite services are geared towards improving the participant’s ability to perform the activities of daily living, which include the following:

  • Bathing
  • Dressing
  • Eating
  • Mobility and instrumental activities of daily living
  • Communication
  • Survival skills
  • Cooking
  • Housework
  • Shopping
  • Money management
  • Time management
  • Use of transportation
  • On-site modeling of behavior
  • Behavior support
  • Intensive behavior episode intervention, training, cueing, and/or supervision
